Every Health Alliance associate who is covered under the Health Alliance Select medical may enroll in AllianceRx: Mail Order. By enrolling, you can save time and money by ordering your Alliance Select formulary maintenance medications online or over the phone and have them delivered right to your door. (Maintenance medications are prescriptions you take routinely and on an ongoing basis. Formulary medications are those covered under the Alliance Select medical plan.)
To participate in the program, just follow these easy steps:
Step 1. Enroll in AllianceRx: Mail Order. Because Alliance Select members are not automatically enrolled in AllianceRx, you must enroll in the program by filling out the AllianceRx enrollment form and faxing or mailing it to the AllianceRx: Mail Order pharmacy. Enrollment forms are also available in all human resources departments.
Step. 2. Arrange payment. Payment must be made before the pharmacy will fill your prescription. The most convenient payment option is MasterCard or Visa (credit or debit card). There’s a place on the enrollment form to fill in credit/debit card account information. For questions about prescription prices or method of payment, call 513-584-1397. You have the option of changing your payment method if needed.
Step
3. Send
your
prescription
to the
AllianceRx:
Mail
Order
pharmacy.
The most
convenient
way to
provide
a prescription
to AllianceRx
is to
have
your
physician
write
a new
prescription
for the
medication,
and then
you mail
the prescription
to AllianceRx.
You are
able
to order
a 90-day
supply
for most
maintenance
medications. Make
sure
your
physician
writes
your
prescription
for a
90-day
supply
with
up to
four
refills.
This
will
allow
you to
get a
90-day
supply,
cutting
down
the number
of times
you'll
need
to order
your
medications,
and ensures
that
you only
have
to request
one prescription
per year.
Step 4. Check your mail. You can expect to receive your medication within 7 business days after the pharmacy receives your prescription or refill request. Medications will be mailed via United States Postal Service directly to your home (or other designated address if indicated on enrollment form).
Step 5. After initial prescription-fill, order refills as prescribed. Once a prescription has been filled by AllianceRx, you may have it refilled by filling out the online refill form. You may also call the AllianceRx refill line at 513-584-2323 to order a refill.
