to remain a part of your care even if he or she cannot follow you to the new long-term care facility. For patients without a primary care physician, we can serve in this role or suggest an appropriate referral.

Additionally, our physicians serve as the medical directors for numerous long-term care and assisted living facilities throughout the region. In these roles, they have direct input in overseeing the quality of care for older adults within these facilities and our community

Our physicians provide ongoing care for patients at many facilities. 

House calls for home-bound patients

Sometimes, elderly patients may have difficulty leaving their homes to seek much needed medical advice or care. The The St. Luke Hospitals offer a Senior Medical House Call Program in which physicians and nurse practitioners visit homebound elderly patients. Patients receive needed care in the convenience and comfort of their own homes, reducing the need to move to a long-term care facility.

Medical clinics

We also are able to bring our services directly into the communities we serve. Our geriatric clinics provide primary care services within selected independent facilities in Ohio and Northern Kentucky.

Our Care Teams

Our interdisciplinary team is comprised of some of the most experienced and respected providers in the region. The team includes physicians and nurse practitioners, all of whom are experienced and specifically trained to work with the special concerns of older adults.

Our nurse practitioners work with physicians to provide direct patient care. They perform many of the same duties, including obtaining medical histories, diagnosing and treating patients, providing patient education and coordinating certain medical and laboratory procedures. This allows for a more “hands-on” approach to patient care.
Our physicians and nurse practitioners provide ongoing care for patients in many
facilities.
