It’s also an approach that respects the value and dignity of each individual and his or her unique medical concerns. We’ll help you and your family understand and find solutions to medical, emotional and social challenges commonly associated with aging. Our Care Team Our interdisciplinary team is comprised of some of the most experienced and respected providers in the region. he team includes physicians and nurse practitioners, all of whom are experienced and specifically trained to work with the special needs of older adults. Our physician assistants and nurse practitioners work with physicians to provide direct patient care. They perform many of the same duties, including obtaining medical histories, diagnosing and treating patients, providing patient education and coordinating certain medical and laboratory procedures. This allows for a more “hands-on” approach to patient care. Our physicians and nurse practitioners provide ongoing care for patients in many facilities.
