The American Heart Association reports that every 29 seconds an American suffers a heart attack and every 53 seconds someone suffers a stroke. A severe illness can be devastating to a family’s finances. Critical Illness Insurance provides financial assistance when you are treated for any of the following specified health events:

Coverage Options

There are two options available to you, your spouse or domestic partner, and your dependent children to age 25 (if full-time student). For the Health Alliance definition of domestic partner, see Your Eligibility for Health Alliance Benefits, page 1.

This insurance is simplified issue which means that you will be asked a short series of questions dealing with health issues covering the previous five years. Pre-existing condition limitations are applied to any illness or injury suffered during the six month period prior to the plan’s effective date. Benefits are payable for pre-existing conditions if the specified health event occurs more than 30 days after the effective date of coverage.

Cost of Coverage

The cost of coverage varies depending on the individuals covered and their ages. Refer to the cost sheet insert for specific premium amounts. Premiums are deducted on a pre-tax basis from every paycheck. However, if your policy includes coverage for a domestic partner, deductions are taken on an after-tax basis.

Personal Accident Expense Insurance Plan

Approximately 40% of all hospital emergency room visits are injury related. Accident expense insurance is designed to pay you directly, regardless of any other insurance you may have. This added protection provides 24 hour coverage to help defray the cost of out-of-pocket expenses resulting from such incidents.

Coverage Options

Coverage can be purchased for you, your spouse or domestic partner, and dependent children to age 23 (if full-time student). For the Health Alliance definition of domestic partner, see Your Eligibility for Health Alliance Benefits, page 1. Benefits provided by this policy include:

Other specified injury and miscellaneous benefits payable under a pre-determined payment schedule include:

  • Burns ($100 - $10,000)
· Dislocations ($50 - $2,000)
  • Fractures ($100 - $2,000)
· Lacerations ($50 - $400)
  • Ruptured discs ($500)
· Tendon & ligaments ($500)
  • Out-patient blood and plasma ($100)
· Physical therapy ($25 - up to 6 per accident)
  • Transportation out of area (up to $300)
· Prosthesis ($100 - $500)
  • Ambulance ($100 to $500)
· Family lodging if over 100 miles ($100 per night)
  • Accidental Death and Dismemberment
    (from $250 for the loss of a finger to $100,000 for death in a common carrier accident
  • Wellness benefits - $60 per year (benefit allowance offered after insurance is in effect for 12 months).

This insurance is guaranteed issue. This means that there are no underwriting requirements or pre-existing condition limitations.
