For access to LASTWORD in an Alliance Primary Care office, please follow the instructions below:
- Please fill out a LASTWORD Access and Training Request form on the Intranet. To access the form, select IS&T Services from the Job Tools list. Call the UC Health Service Desk at 513-585-6972 if you have additional questions.
- If you are currently on the UC Health network, click here to go to the IS&T Services request page.

For access to LASTWORD from any other physician office, please follow the instructions below:
- Print out the following four forms:
- LASTWORD User Access Form
- Confidentiality and Data Security Agreement
- HIPAA Addendum
- Statement of Certification
- Fax completed forms to LASTWORD Security at 513-585-7193.
- Print out the LASTWORD Review only Manual.
