The PGY-1
Resident
at St Luke
Hospitals and the
PGY-2 Primary Care
Specialty Resident
must obtain
dual licensure.
The information
provided
below outlines the
method for obtaining
licensure in both
Kentucky and Ohio
and provides the
websites where additional
information is available.
Financial assistance
for dual licensure
may be requested
by the resident.
Kentucky Requirements
Licensure
by Examination:
- Internship hours:
At least
1500 hours
- Must have an approved HIV/AIDs certificate
- Submit three application:
- NAPLEX Registration Form: application fee $430.00*
- MPJE Registration Form: application fee $150.00*
- Kentucky Application for Pharmacist Examination: application fee $150.00**

Examination administered in January and July
Licensure by Reciprocity:
- Must have a valid pharmacist license for one year
- Must have worked 1500 hours in the previous year
- Submit a licensure transfer application(available at NABP.net): application fee $250.00
- Complete the HIV/AIDs CE provided by the Kentucky State Board of Pharmacy
- Complete the application MPJE application: application fee $170.00
Ohio Requirements
Licensure by
Examination:
- Internship hours: At least 1500 hours
- Submit three applications
- NAPLEX Registration Form: application fee $430.008
- MPJE Registration Form: application fee $150.00*
- Ohio Board of Pharmacy Application: application fee $110.00**
Licensure
by Reciprocity:
- Hold
