Pharmacy Residency Requirements

• Doctor of Pharmacy degree
• Eligible for pharmacy licensure in the state of Ohio and/or Kentucky (depending on preferred site)
• Dual state licensure requirements
• Participation in the ASHP Matching Program
• The Pharmacy Residency at the Health Alliance is recognized and accredited by ASHP as one residency program for the entire health system. However, there are three different sites that are available for the Pharmacy Residency. These sites include:
  

  Site	Match Number
  The Jewish Hospital (Cincinnati, OH)	8025
  The St. Luke Hospitals (Northern Kentucky)	8026
  The University Hospital (Cincinnati, OH)	8023

• Each site has a different match number but the same application must be filled out for each. The preferred primary site must be specified on the application.
